In the time between our phone call and your appointment, it would be helpful if you are able to identify some goals and expectations for your therapy. Once you arrive for your appointment, you will be asked to fill out brief paperwork with some information about yourself, such as, your name, address, phone number, your goals and expectations.
All that you need to bring with you to your appointment is your payment, insurance card or EAP autorization number, and an openness to think about, feel or do things differently to create the change you seek in your life. Look forward to working with you.
